Your First Visit

On your first visit, information will be gathered and an assessment will be done to assist in the development of your treatment plan. Your goals for therapy will be discussed.

Please bring your insurance cards and a list of your current medications. If you have a deductible and/or copay through your insurance, please bring this information with you. Also check with your insurance co. about any preauthorization.
